What You'll Do:

As a Social Worker, you will be a dedicated advocate for our residents, ensuring their holistic needs are met. Your role will be essential in enriching their quality of life and overall well-being.

Key Responsibilities of a Social Worker:

  • Conduct thorough psychosocial assessments to evaluate residents' social, emotional, and environmental needs, along with their strengths and preferences.

  • Collaborate with residents, families, and the interdisciplinary team to develop personalized care plans that address social, emotional, and psychological well-being.

  • Provide counseling and emotional support to residents and their families, offering guidance on coping strategies, care transitions, and decision-making.

  • Advocate for residents' rights and preferences, ensuring their voices are heard and respected in care planning and decision-making.

  • Connect residents and their families with community resources, support services, and advocacy organizations to address social, financial, and legal concerns.

  • Offer crisis intervention and support during difficult situations, such as grief, family conflicts, or changes in health status.

What You'll Bring:

Qualifications of a Social Worker:

Current LSW licensure in the state of Massachusetts.

Bachelor’s degree in Social Work from an accredited program (Master’s degree preferred but not required).

Experience in long-term care, geriatrics, or a related field.

Strong skills in assessment, counseling, and crisis intervention.

Excellent communication and interpersonal abilities.

Ability to collaborate effectively within a multidisciplinary team.

Empathy, compassion, and a commitment to person-centered care.
