Position Overview: The Mental Health (MH) Outreach Worker will be part of the Mobile Mental Health Team in Clinical Programs.
Position Responsibilities: This position will follow up on new referrals for Mental Health (MH) support through home visits, intake assessments, and short term (6-9 months) support for older adults, 60 years old and over. The outreach worker will create care plans, complete referrals and document visits in the MH documentation system. The Outreach Worker will also collaborate with other providers and build a working relationship with clients to best assess their long-term support needs.
Qualifications:
Bachelor's degree in human services plus 1 (one) year experience OR Associate's degree plus 2 (two) years of experience working with behavioral health consumers in an outreach or community based setting
Bilingual Spanish preferred
Ability to process information accurately
Proficient in the use of computer systems, including but not limited to, Windows based software application including but not limited to Word, Excel, PPT and Outlook
Ability to work both independently and as part of a team
Professional demeanor and ability to handle confidential materials with discretion
Ability to build positive relationships with isolated older adults
Must have reliable transportation and valid driver's license in good standing
Hours Full Time 40 hours a week OR Part Time 20 hours a week, Monday through Friday
