Summary This Social Worker (Suicide Prevention Case Manager) within the Mental Health Service located at the VA Bedford Healthcare System. The position is full-time at 40 hours per week. Responsibilities The principal role of the Suicide Prevention Case Manager (SPCM) is to coordinate and monitor services for Veterans with a Category I Patient Record Flag-High Risk for Suicide (PRF), and act as the transitional coordinator of care for these patients. VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Total-Rewards-of-an-Allied-Health-VA-Career-Brochure.pdf Social Work: vacareers.va.gov/wp-content/uploads/sites/5/Total-Rewards-of-a-Social-Worker-Career-Flyer.pdf Duties include, but are not limited to: Provide support to Veterans with a PRF during inpatient hospitalization including but not limited to individual sessions, phone contact, and discharge planning; Collaborate with Inpatient MH treatment providers to support appropriate discharge planning and treatment for patients at high risk for suicide; Provide support to assigned Veterans with a PRF, including but not limited to, introduction to Suicide Prevention, assess needs, identify appropriate interventions, provide brief, solution focused therapy, provide case management services; Facilitate or co-facilitates group therapy using EBP as assigned; Collaborate with Outpatient/Residential MH treatment providers to support appropriate treatment for Veterans with PRF; Ensure Veterans with PRF receive requisite and clinically appropriate mental health appointments within 30 days of designation as high risk for suicide; Monitor daily no-show report and ensures follow up in accordance with high-risk requirements is completed. Provides support to providers and Veterans following missed appointments as needed; Ensure Veterans with PRF has a documented safety plan in the electronic medical record; Complete a minimum of 4 contacts in CPRS documenting SPCM involvement in Veterans care, including initial contact within 5 business days of PRF being activated, 30-Day, 60-Day, and 90-Day review; Complete a full review 90 days after PRF activation date which includes summary of care, progress, current life stressors, risk factors, protective factors, and recommendations; Consults with Veterans care team as needed to determine inactive/continue of PRF; Upon completion of 90-Day review SPCM will continue/inactivate/transfer PRF within review month; Update Suicide Prevention SharePoint tracking tools daily to reflect completed case management tasks; Complete clinical documentation in CPRS within 24 hours of contact; Ensure encounter information is completed; Update SPAN daily for assigned Veterans. Other duties as assigned. Work Schedule: Full-time, Monday-Friday, 8:00am-4:30pm. Recruitment Incentive (Sign-on Bonus): Not authorized. Qualifications Basic Requirements: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Grade Determinations: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker, GS-11. Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level. OR, Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.
