Seven Hills Foundation Intake/Outreach Coordinator in Worcester, Massachusetts
The Intake and Outreach Coordinator will provide and coordinate all intake and referral activities for Assigned Enrollees of the Massachusetts Care Coordination Network (MCCN) and will be responsible for working closely with the MCCN Director of Integrated Care Management & Coordination to develop and execute a comprehensive marketing and outreach strategy for MCCN’s integrated LTSS care management and care coordination service lines.
The main goals of the MCCN are to support MassHealth Members with complex LTSS needs and their families to help them navigate the complex system in Massachusetts; Improve Member experience and quality of care by assisting them in identifying and accessing appropriate setting and levels of care through care planning, care coordination and support during transitions in care; Support values of Person-Centeredness, Independent Living, aging in place, and cultural competence.
Work collaboratively and effectively with EOHHS, ACOs and MCOs to accept referrals to MCCN and provide proper confirmation following all guidelines outlined in Model Contract.
Serve as the single point of contact for ACO/MCO referrals.
Create Enrollee record in Care Management Tool and assign Enrollee to MCCN Partner based on geography, expertise, and case load of Partner organizations.
Complete all required documentation in a timely manner.
Work collaboratively with the Director of Integrated Care Management & Coordination and the Care Management Software Vendor to ensure smooth implementation, training, and use of tool.
Assist the Director of Integrated Care Management in the development and execution of a comprehensive marketing and outreach strategy.
Develop cultural and linguistically appropriate marketing and outreach materials, social media and web-based tools for the MCCN service line.
Perform all duties in accordance with MCCN’s policies and procedures.
Strictly follow all agency Performance Standards.

B.S. in marketing or related field. Master’s degree preferred.
Five years of experience working in healthcare industry and/or with individuals with complex LTSS needs.
Strongly prefer candidates with experience in managed care organizations.
Ability to use Care Management Software to document and coordinate services.
Must be able to perform each essential duty satisfactorily.
Ability to communicate effectively verbally and in writing.
Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations
Commitment to MCCN values and mission.
Must have valid driver’s license and access to auto.
Ability to read and speak English.
